At WMed Health, we are a teaching facility that offers a team approach to your care. Your care team includes resident physicians who are seeking advanced training, experienced faculty physicians, advanced practice providers, nurses, allied health staff, medical students, and other learners.
Welcome to Our Practices
When you become a patient at WMed Health, during your first visit we will provide you with a new patient packet that includes all the information you need to know about us.
Hospitalization
Our doctors will take care of you if you are admitted to Ascension Borgess Hospital or Bronson Methodist Hospital.
Medical Records
Your medical record is confidential. We will not talk about your condition with anyone without your permission. If you wish to release information from your medical records, complete the Patient Medical Records Request Form and return it to our practice.
MyChart
MyChart is a free and secure tool that gives you access to parts of your medical record through your own computer or smartphone. You can view your medical information, make appointments, renew prescriptions, receive test results, pay your bills, and much more. To sign up, our Registration staff will assist you. MyChart is available for WMed Health patients in English and Spanish.

Your rights and responsibilities as a patient are very important to your care. If you have questions regarding any of these rights and responsibilities, please speak with any staff member.

Meeting the Communication Needs of Our Patients
We provide free aids and services to people with disabilities to communicate effectively with us and free language services to people whose primary language is not English.
Social Work Services
Licensed Medical Social Workers work closely with providers to ensure awareness of psychosocial concerns. They have the ability to help with assessment and referral to community resources to address social barriers, as well as to provide behavioral health support and brief intervention to improve the quality of life for our patients and their families. Your provider may make a referral to request that one of our Licensed Medical Social Workers assist you.
Dietitian Services
Good nutrition is an important part of treating many disease processes. Registered dietitians provide nutrition education and nutrition therapy to promote wellness, prevention of illness and/or disease management for a variety of conditions. They assist individuals from all walks of life make positive food and diet choices, as well as lifestyle changes, to improve their medical condition and health. If you have pre-diabetes or diabetes, a Registered Dietitian or a dietitian certified as a Diabetes Care and Education Specialist will assist you with understanding how your diet affects daily life and blood sugar control. Registered Dietitians are available to all WMed patients as part of the health care team. Patients are initially seen by provider referral with follow up visits on an individual basis.
Care Management Services
Care management coordinators help patients with complex healthcare needs. These registered nurses are a connection to information about your health and to other healthcare resources. Care management coordinators work with you and your provider to help you learn how to take care of yourself, so you can have the best health possible.
Your care management coordinator may:
- Call you at home to check on how you are doing and answer questions you may have.
- Meet with you at appointments, as needed.
- Assist you to coordinate and schedule appointments.
- Help answer questions about your medications.
- Work with you and your family to develop a plan of care for your health.
- Help you find additional help if you are having trouble paying for your medications.
- Work with you to reach the goals you and your doctor have set for your health.
This service is provided to patients as directed by their primary care provider.
Respiratory Care
A respiratory therapist can assist you with your medical needs related to asthma, COPD, and other respiratory conditions, such as cystic fibrosis. With your doctor’s referral, we can provide tobacco cessation information, respiratory assessment, oxygen evaluations, lung function testing, and inhaler evaluation and training. WMed is proud to be a part of the National Heart, Lung, and Blood Institute (NHLBI) Breathe Better Network. Since our initial subcontract with NHLBI in 2018, we have been guided by the COPD National Action Plan in bringing awareness and resources to those in our community living with or affected by COPD. Complaint Resolution
If you have a concern about your care while you are at the practice, please ask to speak to a clinical leader or medical director. They will assist you in resolving your concern while you are here. Presentation of a concern or complaint will not compromise your access to care. If you are not satisfied with the response to your concern or you have a complaint or compliment after you leave the practice, you may fill out a Patient Feedback Form on the WMed Health website or contact the Quality Improvement and Allied Health Manager by calling the patient complaint/feedback hotline at 269.337.6445 to begin a formal complaint or leave a compliment.
Laboratory and Radiology Services
Outpatient laboratory and radiology services at WMed Health are provided at 1000 Oakland Drive for patients whose tests are ordered by a WMed Health physician.
The Laboratory is located on the Concourse Level and is open Monday through Friday from 8:00 a.m. to 5:00 p.m. You can reach our Laboratory Department at 269.337.4480.
The Radiology Department is located just off the first floor lobby and is open Monday through Friday from 8:00 a.m. to 5:00 p.m. You can reach our Radiology Department at 269.337.6232.
